DISCLAIMER
Industry Xperience, LLC reserves the right to remove anyone from any event that is disruptive to themselves or others.
Any staff or faculty member of Industry Xperience, LLC that are associated with this event are not responsible for any lost
or stolen personal property. The choreography taught by Industry Xperience, LLC staff and faculty may not be claimed,
used, taught, or performed anywhere without the expressed, written consent of both Industry Xperience, LLC and faculty.
MEDIA RELEASE
By signing this form, I hereby give Industry Xperience, LLC permission to use my likeness in any and all film, photography,
videotaping, and any other form of media for myself (if signed Participant is 18+) or for both myself and the minor below.
The media aquired by Industry Xperience, LLC throughout the event may be used for lawful promotional and/or
informational purposes without further authorization required. All such media materials shall remain the sole and
exclusive property of Industry Xperience, LLC. The use of video recording devices of any kind is strictly prohibited
during the event. Exceptions will be allowed during the class breaks and the Showcase presentation.
FACULTY
Faculty are subject to change due to professional obligations. Industry Xperience, LLC shall attempt to replace said
faculty with any faculty member with equal experience/credentials. No refunds shall be given based upon these faculty
cancelations or changes for the event.
LIABILITY RELEASE
I agree to release and to hold harmless Industry Xperience, LLC, Steve Ewing, Faculty and its respective
managers, representatives, guest faculty members, from ANY and ALL liability, whether based on statute, contract, tort,
negligence, or otherwise, for any claims, judgements, liabilities, expenses, or costs, including, without limitation, any
attorneys' fees or expenses. I am releasing harmless Industry Xperience, LLC, Steve Ewing and all staff
and guest faculty of the event from any connection with any illness, injury, or death which may incur during intensives
or other activities at the convention. Participant hereby confirms that he/she is in good health, and is physically able to
perform the dance activities that shall take place at the event. Participant understands and agrees that dance can
be strenuous and may cause injury, and in each case, participant assumes the risk of any and all such injuries, and
all resulting consequences from such injury.
MEDICAL RELEASE
I acknowledge and agree that the participation at Industry Xperience involves unique movement which carries a reasonable
risk assumption and or physical illness or injury (minimal, serious, catastrophic and/or death) can result from participating
in this activity/workshop/performance. I/We fully assume all risk and all responsibility for losses, cost, and damages, I/We
incur as a result of my/our participation in the activities Industry Xperience has to offer. I/We hereby release, discharge, and
covenant not to sue and agree to indemnify, save and hold Industry Xperience, LLC, Steve Ewing, Faculty, and all
staff and guest faculty and its respective administrators, managers, agents, officers, volunteers, other participants, and
sponsors, advertisers, and if applicable, owners, or leasers, or premises on which the activity takes place. I/We or
anyone on my/our behalf makes a claim against any of the above "releases" I/We will indemnify, save and hold harmless
each of the "releasees" from any litigation expenses, attorney fees, loss liability, damages, or costs that may incur as the
result of any such claims. I/We state that I/We have not been exposed or in contact with persons positive for Covid-19.
I/We agree to wear a mask, social distance and obey all health and safety protocols while on-site during the event.
I hereby warrant that I have read the above liability release form in its entirety and fully understand its contents.
I have read and understood the assumption of all risks, release of indemnity agreement, as well as the medical waiver.
I agree to adhere to ALL policies set forth by this waiver. I have digitally signed this document voluntarily and of my own free will by completing this registration.
